Pre-Registration Terms and Conditions for Agarwal Wellness Medical Centre

Thank you for your interest in registering as a new patient with Agarwal Wellness Medical Centre Lucan. To make sure we can give the best care to our local community, please review and agree to these pre-registration terms and conditions:

Residency Requirement:
To be eligible for pre-registration, you must live within the Lucan area. We might ask for proof of address when you fully register to confirm this.
Existing GP Registration:
You shouldn’t currently be registered with another General Practitioner (GP) practice in Lucan or the immediate surrounding areas. Our goal is to serve patients who truly need a local GP service. If you’re registered elsewhere, you’ll need to officially de-register from that practice before you can complete your full registration with us.
Appropriate Use of Services:
Agarwal Wellness Medical Centre is committed to providing thorough and responsible medical care. We have a strict policy about the misuse of prescription medications. Because of this, we don’t accept pre-registrations from people who might be seeking drugs. Our medical team can decline full registration if, during the pre-registration or initial consultation, we have concerns about the appropriate use of our services regarding controlled substances.
Medical Record Transfer Responsibility:
Once you’ve been accepted as a patient into our practice, it’s your responsibility to arrange for the transfer of your medical records from your previous GP. We can provide guidance on this process, but the actual transfer is up to you.

By going ahead with pre-registration, you acknowledge and agree to these terms and conditions. Meeting these criteria doesn’t guarantee full registration. That will depend on a review of your medical history and an initial chat with one of our healthcare professionals.
